The Future Health Needs of Shropshire: Expressions of Interest
Residents and organisations are being invited to take part in a workshop and have their say on the changing health and social care needs of the Shropshire population.
The Shropshire Care Closer to Home Team will be leading a Joint Strategic Needs Assessment (JSNA) workshop on Wednesday, 4 December, 2019.
Already attending the workshop will be a range of stakeholders who have been working on the Care Closer to Home Programme and we now have a limited number of free places available. The Programme Team is offering these places to anyone interested in attending and is looking for expressions of interest from any individuals, groups or organisations.
If you would like to attend the workshop, please email firstname.lastname@example.org before Thursday, 31 October, 2019 with your name, details of the group or organisation you represent, and your full contact details. Spaces will be limited and invitations will be allocated on a random basis should the workshop oversubscribe.
As well as influencing the work currently underway by the JSNA, the session aims to collaboratively design the future models of care for Shropshire and review its community bed provision.
The event will also provide an opportunity to deliver a progress update to attendees on the Shropshire Care Closer to Home Programme. This will include the Frailty Intervention Team (FIT), Case Management, and heightened levels of care in the community – previously delivered in hospital.
