Tell us what matters to you for the planning of the new Shropshire Care Closer to Home Project

What matters to people is the question Shropshire Clinical Commissioning Group will be asking local people to help shape and develop the new Shropshire Care Closer to Home Service.


It is holding two informal drop in-sessions in Bridgnorth and Oswestry this Wednesday, 6 June, to chat to local people about the Shropshire Care Closer to Home Project. This new project is for local people over 65 who have a number of health conditions and receive regular care and support from the NHS and Shropshire Council. It aims to help them get their care in the community and even in the comfort of their own homes which could also mean they may avoid a stay in hospital.


The Shropshire Care Closer to Home Team is developing the new service to join up health and social care. They will be on hand to talk about the planned new service and hear feedback and ideas to help develop it.


They want to know what matters in people’s day-to-day experience of the care and support they receive from the local NHS and Shropshire Council and if there is anything that could be changed.


Dr Jess Sokolov, Clinical Lead for the Shropshire Care Closer to Home Project, said: “We’ve already been talking to our GPs and health and social care partners about how we can all work together to support patients so they can avoid unnecessary hospital stays and get their care in their own home which helps to improve recovery.


“We are now holding these two pilot events and want to speak to local people for their feedback as we are at the very early stages of developing how the service could possibly look in the future.”
