Shropshire CCG - Shropshire Young Health Champions

The Shropshire Young Health Champions project was set up by Shropshire Clinical Commissioning Group and is a way of supporting younger people to have a greater understanding of health issues and how they can take responsibility for their own health and wellbeing.

A short film has been written, directed and performed by the Shropshire Young Health Champions with the support from Painted Life Productions. Their aim is to provide information for schools to enable all young people with diabetes type 1 to feel safe and happy at school. Their video is available on You Tube.

Shropshire CCG - Managing Your Diabetes Booklet

This booklet outlines help to manage diabetes.

Shrewsbury and Telford Hospital NHS Trust

One of the two Diabetic local services providers is the Shrewsbury and Telford Hospital NHS Trust.

The Shrewsbury and Telford Hospital NHS Trust is the main provider of acute hospital care for almost 500,000 people from Shropshire, Telford & Wrekin and mid Wales. 

The in-patient diabetes specialist nursing (DSN) team focus on patient support and education whilst also supporting staff and improving care standards. They also liaise with other health care professionals when required.

Shropshire Community Health NHS Trust - Diabetes Specialist Nursing Service

One of the two Diabetic local services providers is the Shropshire Community Health NHS Trust.

Shropshire Community Health NHS Trust provides a range of community-based health services for adults and children in Shropshire, Telford and Wrekin, and some services to people in surrounding areas.

The diabetes specialist nursing team are a clinically-led service that provides care to adults and children with type 1 diabetes, or those with type 2 diabetes who manage their diabetes with Insulin or who are unable to control their diabetes with tablets alone and require injections.

Shropshire Community Health NHS Trust - Diabetes Specialist Nursing Service

STILE Diabetes Programme Course

STILE is run one day a week for four weeks. It is for people with Type 1 diabetes who inject rapid-acting Insulin with meals and long-acting Insulin, 1 to 2 times a day. You will be taught how to work out the amount of carbohydrate in the food you eat and then to use this information to calculate how much Insulin to inject.

This is run by Shropshire Community Health NHS Trust’s Diabetes Specialist Nursing Service.

This allows people at risk of, or diagnosed with, diabetes, to have access to good quality and enjoyable structured education leading to increased skills, knowledge and confidence in the prevention and self-management of their long-term medical conditions.

Shropshire Council

Shropshire Council is the local authority of Shropshire (excluding Telford and Wrekin). It is also one of the local services providers.

Shropshire Council - Community Directory

Shropshire Community Directory is your local gateway to information on over 2,500 community groups, clubs, societies, support groups and self-help groups throughout Shropshire.

Locally, the National Health Service, Local Authority, Voluntary Agencies, patient representatives and members of the public have combined to help produce an awareness of the local Self Help Groups available for different Long Term Health Conditions, under the banner “Self Help Shropshire.”

The health related Self Help (Peer Support) groups are made up of people with a particular health condition and those that care for them (carers).

They are able and willing to share their experiences and knowledge to offer practical and emotional support to each other, providing valuable support and often based in the local area.

A group meeting format varies and can include a guest speaker, a social element and a time to ask and answer questions.

The Wise & Well Team organise information events in towns and large villages across the county, so that people can access quality advice in their local community. These are in partnership with other services and voluntary organisations to provide information about a broad range of relevant topics.
