About the Team

The main focus of the Medicines Management Team is on drug optimisation, prescribing, audits and practice project work to support reductions in hospital admissions. The pharmacists will often perform medication reviews for patients and give advice on prescribing from a clinical and value-for-money perspective.

Practice-based pharmacists and technicians come from a variety of backgrounds and have a wide range of skills and expertise. GP Practices are allocated pharmacists and technicians who visit them on a regular basis.

Members of the team also work with Care Homes. Ceri Wright is Care Homes Medicines Management Officer and ensures all the care homes’ medicines managements needs are met. Notably, with the help of the speech and language team, she has refined the use of oral nutritional supplements (sip feeds) and food fortification in care homes. Ceri has developed a Royal College of Nursing (RCN) accredited ‘Medication In Care ©’ training course for registered nurses and carers. This is now, in many care homes, part of their mandatory training. Practice-based pharmacists and technicians also perform medication reviews and checks for care home patients under the direction of Ceri and by request of the home or GP practice. 

Practice-based pharmacy technicians are trained to NVQ Level 3 standard or equivalent and are registered with the General Pharmaceutical Council. They play main roles in auditing and drug optimisation in practices as well as working closely with practices to focus on specific areas, such as waste management, repeat prescribing or patient group interaction.
