Community Services

Overview

Community Services allow people with health needs to stay as independent as possible, often living in their own home.

Shropshire CCG works closely with and commissions services from Shropshire Community Health NHS Trust.

The Trust’s services include district nursing and health visiting.


Shropshire Public Health Nursing Service (PHNS)

The Health Visiting and School Nursing Service has now moved to a single point of access, incorporating health visiting, family nurse partnership and school nursing and is now called the Shropshire Public Health Nursing Service (PHNS).

Community Hospitals

The Community Trust also runs four Community Hospitals in Shropshire.

There is a community hospital in Bishop’s Castle, Bridgnorth, Ludlow, and Whitchurch. There is also a Health Centre in Oswestry.

The Bridgnorth, Ludlow and Whitchurch hospitals have a Minor Injury Unit (MIU), as does Oswestry Health Centre. The MIUs offer help with a range of injuries and ailments including sprains and minor fractures, minor head injuries, burns and even emergency contraception. The Community Trust provides some very specialist community care ranging from dementia care to allergy services (A-Z of services).
